This can happen from time to time. You want to heat up the vial and let it reconstitute itself. Heat can be applied by putting hot water in a cup/bowl and placing the vial in it (water level should be under the rubber top). Let it sit for a minute and then gently swirl the vial.
Unlikely if it's sealed properly and from a reputable source. Sounds like it just crashed a little which happens. You can post pics if you want so we can see it. I'd still try out what I posted previously and see if it clears up.
I resuspend test all the time...I like using a frying pan and heat if to right before boiling when all the small bubbles are appearing at the bottom..set em for 10 minutes and then shake.
